Рецидив после радикальной коррекции врождённого гидронефроза (ГН) у детей возникает в 5–11% случаев. Однако предикторы повторной обструкции пиелоуретерального сегмента (ПУС) ещё недостаточно определены, что обусловливает актуальность темы.</p></sec><sec><title>Цель работы</title><p>Цель работы: определить факторы риска рецидива ГН у детей для разработки методов его профилактики.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Обследовано 100 детей с ГН, которые были распределены на 2 группы: 1-ю группу составили 50 больных с рецидивом ГН; 2-ю (контроль) — 50 детей без рецидива ГН с сопоставимым функциональным состоянием поражённой почки. Проведён анализ влияния пре-, интра- и послеоперационных параметров на исход первичного вмешательства.</p></sec><sec><title>Результаты</title><p>Результаты. У больных ГН 1-й группы при первичном вмешательстве нами выявлены следующие значимые различия по сравнению с больными 2-й группы: применение нерезекционных пиелопластик; более протяжённая стриктура ПУС; отступ менее 5 мм от края стеноза; натяжение краев анастомоза вследствие недостаточной мобилизации; острый пиелонефрит в раннем послеоперационном периоде. При натяжении тканей анастомоза у больных 1-й группы проводился только один вид отведения мочи (внутренний или наружный), а у больных 2-й группы было обеспечено комбинированное дренирование.</p></sec><sec><title>Заключение</title><p>Заключение. Для профилактики рецидива ГН у детей необходимо выполнять исключительно резекционную уретеропиелопластику с адекватной мобилизацией тканей и широким отступом от края стеноза, применять комбинированное дренирование при натяжении краёв анастомоза и обеспечить наблюдение оперированных детей до перехода во взрослую сеть.</p></sec><sec><title>Участие авторов</title><p>Участие авторов: Петрова А.Ф., Коварский С.Л. — концепция и дизайн исследования; Петрова А.Ф., Матвеенко Е.Н. — сбор и обработка материала; Петрова А.Ф. — статистическая обработка материала, написание текста; Коварский С.Л., Меновщикова Л.Б. — редактирование. Relapse after radical correction of congenital hydronephrosis (CH) in children occurs in 5–11% of cases. However, the predictors of repeated obstruction of the pyeloureteral segment (PUS) have not yet been sufficiently evaluated, which determines the relevance of the topic.</p></sec><sec><title>Aim</title><p>Aim: to detect the risk factors for recurrence of hydronephrosis in children to develop methods for its prevention.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. There were examined one hundred children with hydronephrosis divided into 2 groups. Group 1 consisted of 50 patients with recurrent hydronephrosis (RH), group 2 (control) — 50 children without RH with a comparable functional condition of the affected kidney. The analysis of the influence of pre-, intra- and postoperative parameters on the outcome of the primary intervention was carried out.</p></sec><sec><title>Results</title><p>Results. During primary intervention in CH patients from group 1, we identified the following significant differences compared with patients of group 2: the use of non-resection pyeloplasty; a more extended stricture of the PUS; an indentation of less than 5 mm from the edge of stenosis; tension of the edges of the anastomosis due to insufficient mobilization; acute pyelonephritis during the early postoperative period. When the anastomosis tissues were stretched, only one type of urine drainage (internal or external) was performed in patients from group 1, and combined drainage was provided in patients of group 2.</p></sec><sec><title>Conclusion</title><p>Conclusion. To prevent recurrence of hydronephrosis in children, it is necessary to perform exclusively resection ureteropyeloplasty with adequate tissue mobilization and a wide margin from the edge of stenosis, use combined drainage when stretching the edges of the anastomosis and ensure the observation of operated children before moving to the adult network.</p></sec><sec><title>Contribution</title><p>Contribution: Petrova A.F., Kovarskiy S.L. — research concept and design of the study; Petrova A.F., Matveenko E.N. — collection and processing of material; Petrova A.F. — statistical processing, writing the text; Kovarskiy S.L., Menovshchikova L.B. — editing the text.
