Потеря веса преимущественно за счёт снижения мышечной массы часто является коморбидным состоянием пациентов при тяжёлых заболеваниях и травмах, тесно связана с неблагоприятным прогнозом течения заболевания и развитием осложнений, ограничивающих последующую активную реабилитацию. Значительным препятствием в анализе данных о развитии саркопении у пациентов с последствиями травмы является отсутствие консенсуса относительно её определения и диагностических критериев.На современном этапе оценка состояния компонентного состава тела пациентов ограничивается антропометрическими и лучевыми методами, которые позволяют констатировать лишь факт развития саркопении, вместе с тем изучение прогностических биомаркеров и определение биологических фенотипов травмированных пациентов позволяют фиксировать тонкие изменения в метаболическом профиле организма на ранних сроках формирования патологических процессов. Например, плазменный метаболомный профиль может напрямую отражать метаболические нарушения, связанные с мышечной атрофией, поскольку плазма является основным переносчиком метаболитов.В обзор включены исследования, посвящённые лабораторной оценке состояния нутритивного статуса, скелетной мускулатуры пациентов, перенёсших тяжёлую травму, а также экспериментальные исследования на животных с определение уровня отдельных аминокислот плазмы крови и их влияния на состояние мышечной массы.Настоящий научный обзор подтверждает необходимость выявления ранних диагностических критериев, важных для мониторинга состояния компонентного состава тела пациентов с тяжёлой травмой, с целью разработки новых терапевтических стратегий, направленных на коррекцию выявленных нарушений.</p></abstract><trans-abstract xml:lang="en"><p>Currently, the issues of sarcopenia and/or developing sarcopenic obesity in patients with severe injuries are receiving increasing attention. Weight loss, primarily due to reduced muscle mass, is a common comorbid condition in patients with severe illnesses and injuries and is closely associated with an unfavorable disease prognosis as well as with complications that may limit subsequent active rehabilitation. A major obstacle to analyzing sarcopenia development following trauma is the lack of consensus on its definition and diagnostic criteria.At present, assessment of patient’s body composition is limited to anthropometric and radiological modalities, which only allow confirmation of the presence of sarcopenia. However, identification of prognostic biomarkers and biological phenotypes in injured patients allow to detect mild metabolic changes at the early stages of pathological processes. For example, the plasma metabolomic profile can directly reflect metabolic disorders associated with muscle atrophy, as plasma is the main carrier of metabolites.
