Оценка только антропометрических параметров не позволяет судить об изменениях компонентного состава тела, в то время как дефицит безжировой и скелетно-мышечной массы тела при хронической патологии является фактором риска тяжёлого течения основного заболевания. &#13;
Цель: определить особенности физического развития и компонентного состава тела детей с БК. &#13;
Материалы и методы. Обследован 121 ребёнок в возрасте 7–17 лет (медиана возраста 15 лет) с подтверждённым диагнозом БК. Всем детям проводили антропометрические измерения, анализ компонентного состава тела методом биоэлектрической импедансометрии и определение индекса активности заболевания по шкале PCDAI.&#13;
Результаты. У обследованных детей выявлена нутритивная недостаточность в 55% случаев. При оценке состава тела дефицит безжирового компонента был установлен у 46% обследованных детей, недостаток скелетно-мышечной массы — у 39,7%, Z-score жировой массы был снижен лишь у 6,6% пациентов, что свидетельствует о её повышенном содержании независимо от индекса массы тела. Установлена слабая обратная корреляция между значениями индекса активности заболевания PCDAI и показателями безжировой (rxy = –0,331; p &lt; 0,001), скелетно-мышечной массы (rxy = –0,359; p &lt; 0,001) и фазового угла (rxy = –0,356; p &lt; 0,001). Значимая корреляция между значениями Z-score индекс массы тела/возраст (rxy = 0,856; p &lt; 0,001) и Z-score безжировой массы (rxy = 0,764; p &lt; 0,001) была выявлена с окружностью средней трети плеча. У пациентов с активной БК по сравнению с пациентами в ремиссии установлено уменьшение безжировой, скелетно-мышечной и активной клеточной массы, а также показателя фазового угла.&#13;
Заключение. При определении нутритивного статуса детей с БК необходима комплексная оценка антропометрических показателей с измерением роста, массы тела, окружности средней трети плеча и толщины кожно-жировой складки и расчётом их индексов, а также параметров состава тела.</p></abstract><trans-abstract xml:lang="en"><p>Introduction. Children with Crohn’s disease (CD) are at high risk of developing nutritional deficiencies. Assessment of anthropometric parameters alone does not give us enough information to judge about changes in body composition, while the attention to the problem of deficiency of fat-free and skeletal-muscular body mass in chronic pathology, which may serve as risk factors for a more severe course of the disease, is increasing. &#13;
Materials and methods. The study included 121 child diagnosed with CD. All children underwent anthropometric measurements, assessment of body composition by bioelectrical impedance analysis and disease activity index according to the PCDAI scale.&#13;
Results. Nutritional deficiency was detected in 55% of the examined children. When assessing body composition, a deficiency of the fat-free mass was detected in 46% of the examined children, 39.7% had a deficiency of skeletal muscle mass, while Z-score of fat mass was reduced in only 6.6% of patients. Thus, a relative increase in the fat component of body composition was detected in the examined patients. A statistically significant inverse correlation of the PCDAI disease activity index was observed with fat-free (p &lt; 0.001; rxy = –0.331), skeletal muscle mass (p &lt; 0.001; rxy = –0.359) and phase angle (p &lt; 0.001; rxy = –0.356). No correlation was obtained with fat mass content. The closest correlation with BMI (p &lt; 0.001; rxy = 0.856) and fat-free mass (p &lt; 0.001; &#13;
rxy = 0.764) Z-scores was found for the mid-upper arm circumference. In active CD patients fat-free mass, skeletal-muscular mass, active cellular mass, and phase angle were significantly lower than in those in remission. &#13;
Conclusion.
